Fix inaccuracies in test format documentation. 

* `block_header` operation to accept input of type `BeaconBlock`
* `voluntary_exit` operation to accept input of type
  `SignedVoluntaryExit`.
* sanity/blocks to note that inputs are of type `SignedBeaconBlock`
