align BeaconState docs with latest spec (#4884) ...

Only comment changes:

- Bump refs to final v1.3.0 spec
- Align documentation style in various `BeaconState` structures
- Add `justification_bits` / `historical_roots` comment from spec
- Remove `previous_justified_checkpoint` from non-phase0 (same as spec)
- Cleanup some `Modified` tags
