Like #3089, this PR drastially speeds up historical REST queries and
other long state replays.

* document sync committee hack with head block vs sync message block
* add batch signature verification failure tests

Zahary Karadjov ba42b2b316 Correct implementation of the /validator/duties/sync/{epoch} API
According to the spec, this call should return the positions of
the specified validators within the sync committee. The existing
code was instead returning the indices of the sync sub-committees
where the validator is a member.

* consolidate function naming in BeaconChainDB, use some generics
* import `forks.nim` from other spec modules and move `Forked*` helpers
around to resolve circular imports
* remove `ForkedBeaconState`, use `ForkedHashedBeaconState` throughout
(less data shuffling between the types)
* fix several cases of states being stored on stack in tests, causing
random failures on some platforms
* remove reading json support from ncli - this should be ported to the
rest json reading instead (doesn't currently work because stack sizes)

introduce strong typing for sync committee periods (#3040)
The sync committee period used to be a plain `uint64`. With the light
client sync relying more heavily on them, it makes sense to introduce
a proper type for them, similar to how they are already used for `Slot`
and `Epoch`. This introduces such a `SyncCommitteePeriod` type.
Furthermore, some usage code dealing with those periods is cleaned up.

So far, the REST config response did not include all spec constants.
The spec for `/eth/v1/config/spec` defines that the response should
include constants for all hard forks known by the beacon node. This
patch extends the corresponding response to include more constants.

Etan Kissling f5791122f6 widen allowed specs for validator client
The validator client was only able to connect to beacon nodes exposing
the exact same set of spec constants that are locally known via their
config/spec REST API. However, that set of spec constants is dynamic.
As the validator client only requires a subset of relevant constants,
this may lead to compatible specs being rejected. This patch widens the
allowed specs by only verifying that the required set of constants are
present in the spec response, ignoring any spec constants that are not
locally known, and ignoring missing spec constants that are locally
known but not included by the remote beacon node when not relevant for
operation of the validator client.

enable uint128 / uint256 SSZ serialization tests (#2960)
So far, SSZ tests for `uint128` and `uint256` integers were disabled
due to "compile-time issues". The code has been adjusted to support
those additional test vectors, and the corresponding tests have been
enabled.

disentangle eth2 types from the ssz library (#2785)
* reorganize ssz dependencies

This PR continues the work in
https://github.com/status-im/nimbus-eth2/pull/2646,
https://github.com/status-im/nimbus-eth2/pull/2779 as well as past
issues with serialization and type, to disentangle SSZ from eth2 and at
the same time simplify imports and exports with a structured approach.

The principal idea here is that when a library wants to introduce SSZ
support, they do so via 3 files:

* `ssz_codecs` which imports and reexports `codecs` - this covers the
basic byte conversions and ensures no overloads get lost
* `xxx_merkleization` imports and exports `merkleization` to specialize
and get access to `hash_tree_root` and friends
* `xxx_ssz_serialization` imports and exports `ssz_serialization` to
specialize ssz for a specific library

Those that need to interact with SSZ always import the `xxx_` versions
of the modules and never `ssz` itself so as to keep imports simple and
safe.

This is similar to how the REST / JSON-RPC serializers are structured in
that someone wanting to serialize spec types to REST-JSON will import
`eth2_rest_serialization` and nothing else.

* split up ssz into a core library that is independendent of eth2 types
* rename `bytes_reader` to `codec` to highlight that it contains coding
and decoding of bytes and native ssz types
* remove tricky List init overload that causes compile issues
* get rid of top-level ssz import
* reenable merkleization tests
* move some "standard" json serializers to spec
* remove `ValidatorIndex` serialization for now
* remove test_ssz_merkleization
* add tests for over/underlong byte sequences
* fix broken seq[byte] test - seq[byte] is not an SSZ type

There are a few things this PR doesn't solve:

* like #2646 this PR is weak on how to handle root and other
dontSerialize fields that "sometimes" should be computed - the same
problem appears in REST / JSON-RPC etc

* Fix a build problem on macOS

* Another way to fix the macOS builds

rework spec imports (#2779)
The spec imports are a mess to work with, so this branch cleans them up
a bit to ensure that we avoid generic sandwitches and that importing
stuff generally becomes easier.

* reexport crypto/digest/presets because these are part of the public
symbol set of the rest of the spec types
* don't export `merge` types from `base` - this causes circular deps
* fix circular deps in `ssz/spec_types` - this is the first step in
disentangling ssz from spec
* be explicit about phase0 vs altair - longer term, `altair` will become
the "natural" type set, then merge and so on, so no point in giving
`phase0` special preferential treatment

REST API client, JSON-RPC cleanups (#2756)
This refactoring puts the JSON-RPC and REST APIs on more equal footing
by renaming and moving things around, creating a separation between
client and server, and documenting what they are - the aim is to have a
simple-to-use base to start from when developing API clients, as well as
make it easier to navigate the code when looking for the legacy JSON-RPC
interface vs the new REST API.

* move REST client, serialization and supporting types to spec/eth2_apis
* REST stuff now starts with `rest_`, JSON-RPC stuff starts with `rpc_`,
more or less
* simplify imports such that there's a simple module to import for both
server and client
* map REST type and proc names to yaml spec more closely - in
particular, reuse operation and type names in `rest_types` to make
comparisons against spec more easy
* cleaner separation between client and server modules - modules common
between server and client such as `rest_types` and serialization move to
the spec folder - this allows the client to be built with less knowledge
about server internals

Viktor Kirilov 1482b0430d - work towards more REST API endpoints being implemented
- testnets can now be launched with a separate validator client - make altona SCRIPT_PARAMS="--separateVC"
- reverted the ctrl+C signal handler code reuse - not necessary for the VC anyway (default is good enough)
- added a bit more logging in the VC
- removed unnecessary code in the VC - connect() just parses the address & port...
- fixed a couple more VC issues - when fetching the duties for an epoch fails on the BN side ==> the VC shouldn't be left in a broken state
- documented the currently supported json-rpc endpoints
- added more checks on the BN side for the API - bounds-checking the requests & also checking if the BN itself is synced
- other cleanup

currently a local sim doesn't finalize, but participation in the altona network with a separate VC is painless and works just as well as with in-process validators in a BN

Viktor Kirilov 72dfe7f578 - updated the validator shell script after the keystore changes
- better logging & retrying requests on the VC side if the BN fails for some reason
- VC now fetches the attestation duties 1 epoch in advance - in the future it will tell the BN to subscribe to the appropriate attestation topics in advance based on that info
- a bunch of other code cleanup & fixes such as better naming for consoles when using multitail, etc.

reviewed in PR #1184 - proper review of the API & VC are pending

Viktor Kirilov 3bae40ae91 - extracted the commands to run a VC into a separate run_validator.sh script
- each BN gets half of its previous validators as inProcess and the other half goes to the respective VC for that BN - using separate data dirs where the keys are copied
    - also removed a few command line options which are no longer necessary
- block proposals originating from a VC are propagated from one BN to the rest properly
- other cleanup & moving code back to  since it is no longer used elsewhere

Viktor Kirilov 8760494c72 first batch of work towards the VC/BN split:
- we have a new binary which connects via RPC to the respective BN and has an internal clock - waking it up on every slot
- the BN has a new option called --external-validators and currently in order to have the VC binaries to run we need to pass EXTERNAL_VALIDATORS=yes to make
- factored some code out of beacon_node.nim for easier reuse in validator_api.nim and validator_client.nim
- the VC loads its associated private keys from the datadir for its BN
- most of the validator API calls have been implemented as a stub.
- the VC polls its BN at the start of each epoch - getting a list of all active validators for the current epoch - and then continues to request blocks and sign them with its appropriate validators when necessary
