* Remove files before reorg

why
  Using snap/2, some sources will be useless and some others will be
  re-organised. Currently,

* Update/reorg download sub-modules for accounts. header, storage, code

* Update FSA logic and download module

* Update BAL download error handling

why
  Over ethXX/BAL after receiving BAL with does not verify, make sure
  that the same peer over eth is not fetched from, again.

  For snap/2, this situation is still open as snap/2 has not been
  introduced, yet.

* Code cosmetics, comments update, small fixes and updates
