Fix some typos

- defeat "is is" in `vkEnumerateInstanceVersion`
- correct markup doc about em-dash (leftover from #642)
- fix #667 (cmdbuffer initial state mangled sentence)
- fix #687 (`VkFormat::` instead `VkFormatProperties::`)
