Update VkSwapchainCreateInfoKHR descriptions

- fix `pretransform` and `compositeAlpha`; they are values not bitmasks
- update `imageFormat`, `imageColorSpace`, `imageUsage`  and
`oldSwapchain` to be specific to behavior of this command
- fix broken unintended code block
- move `VK_TRUE` clipping text to correct bullet point
- random tweaks...
