For a payment stream, either party (the stream owner or the provider)
may close the stream. Specify that on LEZ the close operation is split
into owner-initiated close and provider-initiated close
(`CloseStreamByOwner` and `CloseStreamByProvider`). Keep Claim owner
non-signing MUST on the provider path. Require distinct provider_id at
create. Change the potential "automatic close on claim" extention to
only apply to provider-initiated close.

On top of this: split withdraw into two commands depending on whether
funds are being withdrawn to owner, or to another address. The reason is
that LEZ doesn't allow repeating an account in the list of in-accounts.
This means that the number of in-accounts differs depending on whether
withdrawal credits the owner's account (same as signer), or a different
account.
