Cut "see X's doc comment for why"-style cross-references throughout —
readers can navigate to referenced items themselves. Also tightened
several overlong paragraphs, and fixed a stale get_program doc claim
(it no longer returns Err(InvalidProgramBytecode) for a corrupted
segment set; that re-verification was replaced by trusting a
non-sentinel current_image_id directly) plus an orphaned comment
fragment left over from an earlier conflict resolution.
